We are pleased to announce an exciting new opportunity to join our expanding Clermont team as a co House Leader on an ongoing basis. 

Stream Services is a registered NDIS provider delivering high quality disability support services across remote and very remote regions of Queensland. We are seeking committed, values driven leaders to support Participants living in a Supported Independent Living (SIL) environment in Clermont. 

Candidates with backgrounds in allied health, nursing, and disability services management are strongly encouraged to apply. 

About the Role 

This is a hands on leadership role for someone who truly understands what quality Participant-centric support looks like and takes pride in building and mentoring a capable, consistent team. 

The role operates on a 7 days on / 7 days off roster, with the House Leaders living and working within the SIL home during each of their rotations, then rotating with the other House Leader. The two House Leaders will work extremely collaboratively and will have opportunity for consultation and planning for the consistent implementation of strategies for the house.   

Supports run day and evening shifts and include primarily inactive sleepovers, ensuring stability, continuity, and strong Participant relationships. 

A core component of this role is direct Participant support, working alongside your team to deliver safe, Participant-centred care. 

Please note: This is not a FIFO role. Due to the responsibilities of the position, applicants must live within reasonable travel distance or be willing to relocate to Clermont. 

Key Responsibilities 

As a co House Leader, you will: 

  • Lead, supervise, mentor, and support a team of Lifestyle Support Workers in a structured, high support group-home environment 

  • Provide direct support to Participants with daily living activities including personal care, meal preparation, and household tasks 

  • Support social, recreational, and community participation to promote independence and quality of life 

  • Oversee the day-to-day operations of the SIL home 

  • Ensure compliance by the team with training, certifications, and organisational procedures 

  • Model professional, consistent, and active support practices that promote safety, choice, and independence 

  • Coordinate daily routines, appointments, and community access in line with Participant goals and risk management requirements 

  • Ensure accurate and timely documentation, including shift notes, incident reports, and participant records in line with NDIS requirements 

  • Contribute to the development and implementation of individualised support plans 

  • Ensure all service delivery aligns with Stream Services policies, procedures, and a strong safety culture 

About You 

You are a confident and compassionate leader who thrives in a residential support environment and leads by example. You will bring: 

  • Formal tertiary qualifications in a disability related field (Allied Health, Health, Community Services, etc), and/or significant direct supervision experience in disability services 

  • Demonstrated experience supporting people with disability, ideally within SIL or shared accommodation settings 

  • Direct experience working with people living with mobility and personal care needs and challenging behaviours 

  • Proven supervisory or leadership experience in disability, health, or community services 

  • Strong organisational and problem solving skills, with the ability to balance multiple priorities 

  • The ability to coach, inspire, and coordinate a dedicated support team 

  • A sound understanding of NDIS Practice Standards, behaviour support frameworks, and Participant-centred active support 

  • Highly developed written communication and administration skills 

  • Friendly disposition, sense of humour, patience and empathy 

Requirements 

  • Relevant qualifications of a Diploma or Degree level or more than ten years experience  

  • Five or more years experience in high intensity and/or group home disability support environment 

  • Current (or ability to obtain) NDIS Worker Screening Clearance 

  • First Aid and CPR certification 

  • QLD Driver’s Licence
