About the opportunity:
This pivotal leadership role is responsible for delivering high quality, person centred home care services that empower older people to live with dignity, safety and independence in their own homes. Leading the day to day operations of a community aged care hub, the role oversees the planning and coordination of individualised care, ensuring services align with each client’s goals, preferences and needs. At the same time, it provides strong leadership and support to a multidisciplinary team, fostering a positive, high-performing culture focused on excellence in quality care.
Blending operational oversight with a genuine commitment to people, the role brings together client advocacy, team leadership, financial management and stakeholder engagement. Working closely with clients, families and health professionals, it ensures every interaction is respectful, responsive and outcomes focused. With a strong emphasis on continuous improvement, service quality and sustainable performance, this role plays a key part in shaping the future of community care within the region.
